Does anyone use a specific client/patient assessment form that is specifically based on Orem's self-care deficit theory?

deejay ... I am currently attending the University of Phoneix and all of our theory based education is focused on Orem's Self-Care Deficit Theory. The book I have used to better understand the theory and learn the assessment principles is Nursing, Concepts of Practice: 5th edition ISBN: 0-8151-6552-8.

This book has really helped me to better understand the theory and attempt to apply it im my practice. I have to admit that while this theory is wonderful and if a nurse had the time to commit to this level of assessment in the "real" world, it would really help to define the patient's deficits. But in reality ... there is simply no time to practice this level of assessment in your day to day dealings with the patient. However, if you are a student like me, then this theory is the basis for learning, I guess.
