P.E.S Nursing Diagnosis

I was wondering if anyone has any suggestions on how to master or even try the skill of writing out a nursing diagnosis. I am having a really hard time just grasping the concept in general!

this may or may not help you, but i've just got the concept down myself and what helped me is when my instructor broke down the pes in questions. p (problem) what is wrong? e (etiology) what caused the problem? s (signs/symptoms) how do i know there is a problem? it's been helping me to just forget the pes part and ask myself those questions. best of luck! :jester:

Swing into a barnes and noble or borders book store and give a look at some of the nursing diagnosis books they have there. They are really helpful. My school had one in the book pack for classes and its helped a ton. And I suppose its like anything else. the more you do it the better you will get

I am a 1st semester nursing student so please correct me if I'm wrong PES is problem(nursing diagnosis), etiology(cause), signs/symptoms used for actual problems so it is a 3 pt. statement. If you have a "risk for" diagnosis it is a 2 pt statement as pt. doesn't have problem yet. For example

Actual: Acute pain r/t Left Total Knee Arthroplasty aeb pt states pain is a 5 on a scale of 1-10.

Potential: Risk for infection r/t LTKA

There are some great stickies at the top of the two student forums that are specific nursing diagnoses and care plan stickies.

The NANDA site is also good. You books may have some guidelines and helpful hints too.

Two things to always remember: 1) it is a NURSING diagnosis not a medical diagnosis and 2) Maslow, Maslow, Maslow. don't forget your priorities.
