Outcomes for impaired skin integrity r/t environmental irritants second

I have to come up with outcomes and interventions for the homeless population. I have not yet worked with the homeless I will be doing footcare. So far I have

Individual will Describes 2 measures to protect their feet from environmental irritants

[TD]1a. Health Teaching

Teach individual ways to protect their feet from environmental irritants (Shoes, socks, keep clean)

1b. Outreach

Provide individual with washcloths and socks to aid in the cleaning and protection of their feet

and

I need another outcome with interventions I have an idea of using

Patient will be able to list the 4 cardinal signs of infection by end of foot wash

[TD]2a. Health Teaching

Teach the patient the 4 signs of infection and explain why watching for this is of importance to them

But I dont feel this is a good outcome

I also have to come up with a goal and so far I have this:

Individual will be able to identify measures in ways to protect their feet from environmental irratants, understand signs of infection, and ........

Please help.
