What kind of sceme do you use to organize your notes so that you can give accurate information when the doctor or other staff members want to know the latest development on a patient. I am new at giving report on 4-6 patients at the end of the shift. I get them a little mixed up or I don't have much to pass on but really do know alot about them but freeze up when it comes time for report. I could use afew tips and ideas on how you organize information with out nessarily writing it down.:trout: Thanks

for starting, I find that it helps me organize information by using the headings:

Hygiene, Elimination, Nutrition, Mobility, Observations, Measurements,

Learning, Person-social

Include specifics: pain, analgesics, sedatives, dressings, vital signs,

IVs, etc

An organizational tip our instructors gave us is to designate a sheet of paper for each patient. When we are given report we right the first notes on the back of the sheet at the bottom. Each time we update the patient progress we write those notes above the previous notes. That way when we give report they are getting the newest information on the patients. It works for me and the nurse taking reports said she really appreciates our students ability to promptly give reports. Also, if you use abbreviations for words it helps. I put my abbreviation code on the back of the paper as well.
