OLDCART

I am a new nursing student and need some help with documenting. Would you oldcart dry skin, cherry angiomas and nevi(moles), and if you do, can you put it in parentheses. For instance Dry hands and feet (OLDCART)

I am also a new nursing student but at my school those would be considered symptoms therefore we would be required to use oldcart. That being said I'm not sure what you mean by just putting it in parathensis. We use OLDCART as a form of symptom analysis so we don't write the word just follow the series of OLDCART questions ensuring they are all answered about the symptom or condition. If that is similar to what you are doing I could possibly be more help :) Good Luck! I need some too..
