OB Nursing Diagnosis question

I am writing up a careplan for my patient and I'm trying to decide if I'm wording one of my diagnoses correctly.

Scenario: I walked into the room and found mom sleeping in the bed (head raised, no rails) with baby beside her and a 4 year old in the bed watching tv.

I am writing up Risk for injury but I just can't figure out how to write up the r/t and AEB. Maybe I'm just over thinking it because it seems too simple.

Anyhow, I have Risk for injury r/t unsafe sleeping practices AEB mom sleeping in bed with baby beside her.

Can I make that any better? It just doesn't seem right to me.

Any advice appreciated.

i would make that the following

  • problem: risk for falls - this is a potential problem of the baby falling out of the bed. this diagnosis includes the falling which is what you want to prevent first. any injury, in this case, would be due to a fall, right? and there is a specific diagnosis that covers that.
  • etiology: mother and baby falling asleep on a bed without side guards this is the reason a fall would occur
  • symptoms: there are no symptoms because this is a problem that does not yet exist. there are only reasons to choose the diagnosis such as observing the mother falling asleep with the baby and other child next to her.

interventions for potential nursing problems ("risk for" diagnoses) are limited to:

  • strategies to prevent the problem from happening in the first place

  • monitoring for the specific signs and symptoms of this problem

  • reporting any symptoms that do occur to the doctor or other concerned professional

an alternative diagnosis to use would be:

  • problem: impaired parenting
  • etiology: (there wasn't enough information provided, but. . .) deficient knowledge of parenting, lack of resources [maybe they have no bed for the baby], sleep deprivation [of the mother. . .what other reason would there be for her to fall asleep and leave this baby in potential danger?]
  • symptoms: mom found sleeping in the bed (head raised, no rails) with baby beside her and her 4 year old in the bed as well watching tv
