OB care plan. Needs help desperately!

Published

So my brain just shut off ...I need some assistance with my OB care plan! I would like to use Risk for Deficient Fluid Volume in relation to her getting rid of her amniotic fluid post-partum and Acute Pain post-partum. Thanks so much for the help...really any nursing dx will do!

When you say "getting rid of her amnoitic solution" do you mean that her membranes (water) broke?

If this is preterm and not the result of labor, I would suggest using "At risk for infection r/t premature membranes ruputured at >>> weeks m/b acute pain"

But more info would help.

postpartum means the baby has been born. was the birth lady partsl or c-section? no, not just any nursing diagnosis will do. nursing diagnosing is serious business. it is determination of the patient's nursing problems and it starts by an assessment of the patient. you must know what birth does to a woman's body. trauma of birth causes things to occur that lead to problems. read about amniotic fluid. it is normally lost the moment it becomes amniotic fluid and it is not absorbed back into the body. on the other hand, the trauma of the baby coming through the birth canal produces the inflammatory response because of the traumatized tissues. the 4 cardinal signs of inflammation in order of occurrence are redness, heat (fever), swelling and pain. because the tissues are internal you will not see the redness and swelling, but i guarantee they are there all along the birth canal. what structures might be affected by this swelling and pain? is this mother breastfeeding? there are 3 diagnoses for breastfeeding and if she is breastfeeding one will apply to her. labor is very tiring and mothers are often very fatigued after labor and delivery and need a day or two of rest to recover. teaching needs also need to be assessed and addressed.
