NY RN license endorsement to illinois

hello,please i need help can anyone guide me how to endorse my ny rn license to illinois,I went to the website IDFPR and i started to fill up the form,I have my NY Credential varification in cgfns do i need to process again CES to CGFNS for illinois?I am confused what to put #33#34#37
