Hi !
I am finishing an NVD care plan. My brain is not working sufficiently anymore. I need some interventions and teaching for fluid volume deficit nursing diagnosis.
Any suggestions ???

look in your care plan book.....why does the patient have NVD. What would you need to teach them if the diarrhea is infectious? Would personal hygiene and hand washing be effective? How would you help dehydration in a patient vomiting? How would you give fluid? What medicines might you give? How would you MEASURE the outcome of your interventions? For example
1) Record intake and output.
rational:
To find out the balance of fluids in the body that are needed for daily metabolism.
2.) Monitor skin turgor.
To find out the less interstitial fluid / loss can lead to loss of skin elasticity.
3.) Observed temperature and mucous membranes.
Dry mucous membranes which is an indicator of dehydration.
4.) Monitoring of urine.
The reduced amount of urine as indicators of reduced fluid in the body.
