Looking for guidelines for end of shift report

I've googled this and also searched these forums, but I'm not coming up with a good list of what exactly should be included in an end of shift report.

Any links or advice would be great.

I've never been able to find any. Everyone has their own ideas about end of shift report. You will find lots of threads about this on the First Year in Nursing Forum. When I gave shift report I generally gave the patient's room number, name, doctor, their medical diagnoses, the last set of vital signs, their blood sugars on my shift, what tests/x-rays had been done on my shift, any new doctor's orders that had been written, and any significant activity that had occurred with the patient's care during my shift. Sometimes there was information about a treatment or medication that needs to be passed on as well. I basically didn't give out information that could be found on the med sheets or Kardex.
