Your goals should be directly related to what you are going to teach the patient.

For short term goals, what do you want the patient to learn and be able to repeat about diabetes before they leave the hospital?

For long term goals, what information do you want the patient to take home and apply to their lifestyle? What will show that the patient has been successful in this? For one long term goal, there is a specific lab result that will show you that the patient has had good glucose control over the past few months. What is that lab value and what result will show that the patient has had good compliance with their diabetic diet?

I'd also take a second look at your R/T of the diagnosis. The patient stated that there was too much information to learn (i.e., he is feeling overwhelmed with the amount of information and seems doubtful that he can learn it all). He did not say that he didn't want to be taught or learn. So I don't think the "lack of interest" is correct.
