nursing ratio's

Published

i am all for the ratio's.. having enough nurses should always be a priority to ensure safe care.. i currently work at a skilled nursing facility in newport beach.. we have 96 beds and have 93 residents in house at this time.. we run with 12 cna's on during the day ... 8 on 3-11 and 5 or 6 on 11-7.. nurses i generally work with 3 during the day 2.5 on 3-11 and 2 on 11-7.. i have been trying to hire nurses for 4 yrs.. rn's don't want to work unless they can be supervisors and do no patient care.. no meds.. or charting.. just admissions.. and orders.. and telling the lvn's what to do.. i won't use rn's that way.. it is a waste of a nurse.. in addition.. we are in a terrible nursing shortage.. there just are not enough nurses.. so.. the question is.. how do you meet your ratio's when there aren't enough nurses with which to do so???.. i ran an add last week.. got one resume!!!!.. this is the usual.. we bring students in to see what long term is like and to possibly have nurses to hire when they are done... we attempt to be competetive.. the nurses that are out there are ones that go from job to job for the money.. and don't care.. soooo again.. how does one meet the ratio????.. any suggestions/?? my ideal is 3-4 nurses on days.. 3 on 3-11 and 2 on 11-7... every time i get close... some one leaves for better pay elsewhere... kathi

Candy,

Thanks for your reply

So I've heard a lot of good things about Mt. St. Mary's... and it's pricey. I'm counting on getting scholarships, grants, loans, and some family help to get me through those 3 years at MSMC.

What kind of nursing do you do?

I anticipate paying my "dues" in a hospital but would like to learn more about the other options in Nursing. It seems to be a growing field with a lot of diversity.

I'm 48 and if I start the progam this fall I will be 52 when I graduate. ALthough I've kept myself in good shape, I may not want to deal with the extreme physical challenges in the hospital.

..... or I could specilize in comatose patients!

we are a single owner facility.. last of a breed.. anything i need we get.. the administrator rarely if ever says not now.. including hiring staff... we do lots of things to keep staff including the fact that i honor all requests as long as it is humanly possible to do so... for the holidays we drew names and gave away turkeys, hams, baskets.. we do taquito's for halloween.. will do something for valentines day.. always coming up with new ideas.. staff is happy and well taken care of.. we were offering $24/hr for a brand new grad. rn with NO ltc experience that we were going to have to orient from the ground up.. the facility offering $32/hr.. can't admit patients.. didn't pass survey..is under new ownership.. another corporation.. has an awful reputation.. but.. i did explain all of this to her.. but..money talks..

Do you hire new grad LVNs? Because if you do, I'm coming in to apply! I'm in Oceanside.

Thanks!

:chuckle AT THE PRESENT TIME NEW GRADUATES ARE COMING INTO SPECIALTY FIELDS IN GEORGIA AT 19.00 HOUR BASE PLUS SHIFT DIFF OF 4.00 FOR NIGHTS AND 3.00 FOR EVENINGS WITH ADDITIONAL 2.00 FOR WEEKENDS. THEY ARE PLACED IN A SIX MONTH ORIENTATION PROGRAM AT OUR FACILITY WITH ONE PRECEPTOR. THEY ARE ATTENDING CLASSES SPECIFIC TO THE SPECIALTY AND AFTER ONE YEAR ORIENTED TO CHARGE AND PAID AN ADDITIONAL 1.75 HOURLY WHILE AN EXPERIENCED NURSE (NEARLY 30 YEARS) IS PAID 27.99 AN HOUR WITH DIFFERENTIALS THE SAME. THE EXPERIENCED NURSE IS EXPECTED TO CARRY A HIGHER RATIO (AS NOT TO FRUSTRATE THE NEW NURSE) AND INTERVENE IN A CRISIS WHEN NEEDED IF THE CORRECT DECISION IS NOT MADE THIS NEEDLESS TO SAY DOES LITTLE TO BUILD THE MORALE OR MAKE FOR A PRODUCTIVE WORK PLACE FOR THE EXPERIENCED NURSE AS SHE IS HAULED INTO THE OFFICE TO ANSWER FOR THE "FOUL UPS ' AND WHY THEY WERE ALLOWED TO HAPPEN.

I LOVE NURSING, LOVE TEACHING AND HELPING THE NEW GRADUATE, I WOULD BE VERY PLEASED TO CONTINUE TO PASS MY KNOWLEDGE ALONG, AND WATCH NURSES GROW IF THEY WOULD ACCEPT ACCOUNTABILITY FOR THEIR ACTIONS, PLACE VALUE ON OTHERS AND NOT FEEL THEY ARE BEING UNDERPAID FOR THE WORK THEY DO. WE ARE GETTING A BUNCH OF FOLKS WHO THINK THEY SHOULD BE PAID TOP DOLLAR FOR SHOWING UP AND MORE IF THEY HAVE TO WORK.

I AM SERIOUSLY LOOKING INTO RELOCATION TO CA TO FIND A PLACE TO WORK WHERE THE SALARY IS GOOD, THE RATIO IS GOOD AND YOU CAN PRACTICE NURSING AS IT SHOULD BE DONE

Hi Shannon,

You will definitely find higher salaries in California, along with a healthy sign-on bonus depending on where you go and what your experience is. I can attest that the Sacramento area is lovely and a wonderful place to live, and the number of hospitals and health care resources here is fantastic. Check out http://www.reallifehealthcare.com for just a taste, and look at http://www.sacbee.com for jobs.

Kaiser Permanente in our region starts RNs at $33 an hour, not including shift differentials. Even a small hospital like Fremont Rideout in Marysville/Yuba City is starting RNs at $25 an hour. If you have experience in preceptorship, you can command a lot more. Aside from housing, the cost of living here is not bad, and new state-wide hospital staffing ratios keep the number of patients to nurses at an even level.

Good luck!
