I am a student nurse and I would very greatful if you would answer these questions.
Age___ Length of time in nursing________ Lenght of time as a charge nurse, manager ,or supervisor______
1. What is the frequency of meication errorsin your area of responsibility?
2. What are your nursing responsibilities when an error occurs?
3. Have you had to develop educational plans or give letters of counseling because of medication errors?
4. What hospital committee makes decisions concerning medication errors?
5. What advice do you have for a new graduate nurse?
6. How are medication errors tracked by nursing to find causes and improve client care?

Length of time in nursing 10 yrs
Lenght of time as a charge nurse, manager ,or supervisor on and off for the last 8 years
Reported med errors happened occasionally, unreported errors probably happen more frequently than I want to think about
I keep a pad of occurence reports nearby and follow up to check for problems with the patient and ensure the reporting process outlined by hospital policy is followed
I don't...... there is the new policy of non punitive med error reporting. I suppose if one amassed enough med errors my manager would counsel them
Pharmacy and therapeutics committee
Make a list and check it twice
Med errors are tracked by the pharmacy and therapeutics committee. each nursing unit has a representative to the committee. Educational endeavors are implemented housewide by the shared governance committee in charge of such things.
