Nursing Diagnosis

I'm working on my care plan and have come up with a hand full of diagnosis based on my assessment. I'm having difficulties prioritizing them. If anyone could give me their input that would be great.

Disturbed sensory perception (tactile)

Impaired oral mucous membranes

Impaired skin integrity

Acute pain

Impaired physical mobility

My top priority nursing diagnosis is going to be:

Acute pain r/t surgical incision aeb patient report of pain report 6/10, facial grimacing, etc....

I was thinking the next 2 priority would be:

Impaired physical mobility r/t musculoskeletal impairment associated with surgery aeb limited range of motion to left lower extremity.

Impaired skin integrity r/t surgical incision

Specializes in Critical Care, ED, Cath lab, CTPAC,Trauma.

without knowing bout your patient and their assessment it's impossible to know which should be considered a higher priority....all care plans are based on patient assessment and needs.

It would not be Impaired Skin Integrity for Surgical Incision. That only includes dermis and epidermis.

I believe the correct one for surgical incisions is Impaired Tissue Integrity.
