I need 3 nursing diagnoses, 1 of which needs to be teaching related for health promotion. With 1 long-term and 1 short-term goal for each. For whatever reason, I am having a hard time writing nursing diagnoses. I have what I want but I'm not sure on how to write it out - ____ related to ____ as evidenced by ____.

-Impaired skin integrity

-Knowledge deficit (teaching one to teach on safe sex and not spreading infection)

-Risk for disturbed body image or Risk for social isolation

My case study:

Allison Jones is a 32-year-old Caucasian female who presents for her routine gynecological exam. She complains of frequent and recurring itching in her lady partsl area along with a burning sensation and pain while urinating. Upon initial assessment, multiple clustered vesicles are present on the patient's inner and outer labia, excessive discharge from the lady parts, and enlarged lymph nodes are noted. The patient's vital signs are: blood pressure 112/68, pulse 62, respirations 14, O2 98%, and temp 99.7 F. Patient states she is currently sexually active with a history of multiple partners. Patient does not use condoms or contraceptives and reports no history of STDs. Patient reports a recent divorce. Patient is currently self-medicating with Tylenol as needed for pain.

I prefer doing the AEB first. Those are your S/Sx. What S/Sx show evidence of impaired skin integrity?

Impaired skin integrity or risk for because of lesions and inflammation? I can add lesions to the case study if needed. It's just made up as we go for a paper we have to write.

I believe if there are vesicles present, it would be impaired, not risk for.
