Don't go at it that way... always leave the nursing dx for the last. Think medically and common-sensically.
Be a mechanic.
What isn't working like it should. Find that "part" - narrow things down. How should it function?
What are the possible reasons for malfunction?
What is/might-go wrong because this "part" is not functioning correctly? -- leads to your nursing dx
("part" can mean anything from body part to cellular activity to medication)

I have a patient who has not passed meconium in over 24 hours, but has no medical diagnosis yet. I'm trying to come up with a diagnosis to match but can find anything anywhere that'll help. she is being fed formula, and everything else is normal, she just hasn't passed meconium yet. Help please. Thanks.
