Is the (evaluation) actual outcome statement based on the interventions? or is outcome statement based on the expected outcome?

If an expected outcome is = Pt will have temp under 99 F by 11/12/08.

Would the actual outcome statement = Pt temp under 99 F by 11/12/08.

Does each expected outcome have a matching actual outcome.

I won't even pretend I know what I'm doing, I just don't. I'm hoping with lots of practice someday I will.

the outcome statement is based on the interventions.

the evaluation is done after the care plan (assessment, diagnosis, outcome and interventions) has been written and implemented. follow the steps pf the nursing process as they occurr. the steps of the nursing process are:

  1. assessment

  2. diagnosis (the problem/s)

  3. planning
    • outcome/goals (what you predict will happen if your interventions are followed)

    • interventions

[*]evaluation (determine if goals/outcomes have been met)
