I'm a first year nursing student.

Please help me with my nursing care plan and nursing report.

I have limited idea about this.

nursing issue: 1. self care deficit related to ageing process

2. Activity intolerance related to unsteady gait

What's the goal for each issue?

what are the interventions for each issue

what are the rationale for the interventions.

is you know some book related to this please tell me...

Secondly, there are several stickies r/t care plans, nursing diagnoses, etc. I suggest you read through these to see if you can find your answers there.

As helpful as we like to be here, we will not do your homework for you. I understand your frustration but at least make an attempt at the work and post what you have. It is easier for us to help you from there.
