Several institutes within the National Institutes of Health in collaboration with the Society of Behavioral Medicine are coordinating an effort to identify a core set of brief, practical measures to recommend for use in adult primary care and public health electronic health records (EHRs). Comments on patient reported measures in EHR will be now accepted through April 11th.

If you haven't done so already, please contribute to this important effort by ranking a set of proposed measures for use in electronic health records (EHRs).

* Click on each construct (e.g., Adherence) to review the measures to be rated.

* Click on the "EHR Campaign Evaluation Form" (red) button.

* Read the summary statement written by the expert panels.

* Click on the PDF icon next to each measure to view the measure.

* Rate the recommended measures and enter your comments.

* Forward this request to your colleagues who may be interested in this initiative.
