I think we need a bit more info.
What clinical area are you working in?
What specifically are you asking about?
Clinical questions, staffing ratios, role of band 5, benefits, training . . . . . ?
Let us know and we can try to answer : )
Hi skylark,
I'm working on cardiology and respiratory ward.
At the minute I'm a bit stuck in sorting out discharge planning for elderly who can't cope home alone, and what are the services available that can help them with this. And who is responsible for organising this,is the nurse in charge or the discharge coordinators.
Cheers
That depends on the set-up at your hospital. Discharge team will generally facilitate discharge by locating a bed or facility, or arranging home care. But the nurse for that patient is usually responsible for the actual transfer, arranging TTOs, transport, faxing notes, etc.
But I would advise meeting with the discharge team yourself and getting their take on it, it might be completely different : )
Thank you,that was most helpful
