newborn nursing dx

Published

HI! I am trying to write 10 nursing dx for a healthy newborn. This is my first time doing one on a newborn so I am very confused. Can anyone help me? pt is a 1 day old female, lost 2 oz since birth, vitals within normal range, bilirubin 2.4, labia barely swollen, milia on nares, no abnormalities, frequent sneezing, breastfeeding well q 1-2 hrs, planned pregnancy, 4th child, Here is what I have:

risk for fluid volume deficit r/t neonatal transition

risk for ineffective thermoregulation r/t immature compensationfor environmental changes secondary to extremes of age

risk for ineffective airway clearance r/t excess fluid in respiratory tract

acute pain r/t birthing process

risk for infection r/t interrupted skin integrity at umbilical cord site

disturbed sleep pattern r/t new environment/ adjusting

and here is where I am trying to make sense:

Effective breastfeeding r/t ?

readiness for enhanced knowledge r/t ?

enhanced bonding r/t mother/infant contact secondary to breastfeeding....is that even a dx?

readiness for enhanced parenting r/t knowledge of dependent needs of newborn....? she had 3 other children

and I have but dont want to use

risk for injury r/t dependence on cargiver.....the mother seems to be good so is there risk?

Help me please! and feel free to critic anything because I am trying to learn and wont if I am not told.

Thank you!

Most of them are acceptable. :)

I'm quite skeptical regarding the "readiness for enhanced knowledge" since the patient is a newborn. Its better if you would apply that to the mother.
