new to QA need a form to QA oasis

In a previous position, I had risk management, safety and quality oversight for hospice and home health service lines, among others. I found these to be useful, but since moving on from that position, I have not kept up with the OASIS regulatory requirements, so check to see if they are still accurate:

Using the forms ensured that we were consistently auditing the charts the same way and helped to establish our due diligence in conducting periodic audits.
