New NP licensing questions

I have a couple licensing questions that I need help with (traumaRN may be able to help with the IL questions).

First, how do I go about getting an NPI number? Do I have to first pass boards and have a license to apply for it? Same with DEA I am assuming?

And licensing: I have received a job for after graduation. I plan on taking my boards the first or second week of May, with a start date of 6/10. I am going to be licensed in IL, and the only application online is the APN Non-Licensure Exam application. I am assuming I fill this one out since I don't need state permission to take the NP exam, and then have my results sent to them. Do I need to pass the boards before I send my application? I am already licensed as an RN in IL, and it asks about work history, license numbers etc. of "current NP licenses or related licenses" so do I have to send my RN licensure proof from ALL states I've ever been licensed in, or do I send none since my RN license isn't a provider license?
