Hello All,
I recently started a new position as a Case Manager for Hospice. I know it will take me some time to learn the in's & out's, but I feel a bit overwhelmed. What are some of the basics to "getting your feet wet" is this area of nursing? I am also anxious about my 1st IDT meeting. How do I prepare?
Any advice is truly appreciated.

Just have a clinical note ready on each patient, just brief notes that give pt status and needs. Your manager should help you out with the first couple of IDG's meetings.

The interdisciplinary team meetings are for collaboration for care that you ANTICIPATE the patient will require in the upcoming 2 weeks. It is the time to discuss the problems on the POC that are not well managed. The discussion should be brief, concise, and problem oriented. Avoid long narratives either verbal or written.
You should prepare by knowing your patients well.
You should be reviewing all POCs and med lists prior to the meeting.
Case Managers are required to provide a written discussion of the case, format and content are defined by the agency within the COPs.
It is reasonable to plan for 1-2 hours/pt for office/case management duties per IDT period (2 weeks). Some patients will not require that much time, some will require much more time. Some of that case management can be accomplished "on the fly", other portions of it demand set aside time.
Hospice case managers practice very individually.
I always prepped my IDT notes in an ongoing fashion, updating them during routine visits...this worked well for me. Some CMs do better having quiet time to gather their thoughts and organize the care. Find your style...

I promise just make sure your med sheets match what the patient is taking and you can still understand energy they are most likely hospice appropriate. You'll be fine.
