New grad LPN, starting job at ALF next week.

I have a few questions on how the average day for the lpn is like in assisted living and when to document certain aspects throughout the day. I currently do not have my license but I'm studying for the nclex and was approved to schedule the date!! I am going to be working as the general nurse, I cannot do blood glucose but I will eventually once I'm licensed. I'm just curious on if the nurse prepares all the meds for the 27 patients on the unit in a certain time slot all at once, or do you go back and forth between each patient? Seems real time consuming, I would like to also know how often to write a nurses note for the patient, do you write a note for every patient every shift? I know that if the pt is c/o pain you have to make a nurses note and then state what your intervention for it was and if it was resolved, I know any stat meds, narcotics and prn meds need to be mentioned in the nurses note as well. Dr calls, pharmacy calls and family visits need to be written in the nurses note as well as if the patient leaves the unit and if physical therapy stopped by etc. do you guys make little notes to yourself with pen and paper when you see the pt then write the paperwork part when you have time? Does your facility have a substance control sheet? Any advice and replays to help me better organize and prepare myself for the job would be so very helpful!
