New grad heading into long term care position...

Hello! I'm a newly graduated R.N. heading into my first position at a long term care facility with a variety of patients (long term care, cardiac, orthopedic rehab, and dementia). They provide 4 days of overall computer and facility training, then I will train on my floor for 3-4 weeks. After that I'll be handling 20-25 patients with a few CNA's working with me. I have a background in medical office positions but obviously I'm nervous working as an R.N. for the first time! Any advice or tips regarding getting acclimated as a new nurse to long term care? Thanks in advance!!
