I do not have a neuro ICU report sheet but I usually have a way of dividing it up with the top as the persons name, Dr, admitting diagnosis and major events during hosptial stay. Then what I do is every system I write on the side and have notes
-Coping
-Neuro-
-Cardiovascular
-Respiratory
-GI
-GU
-Skin
-Function/ ADLs
-Saftey
Then on the bottom I make a task list of things I need to do during my shift and check it every hour, (I write labs on there, glucose checks, deepbreathe and cough, any consultations etc...). After a while some of these things will come natural that you will just learn to keep them in your head.
Here's my current version of the report sheet I use & love. It's in word format so it can be customized to make it work for you!

I work on a surgical/trauma ICU, but it can be used for ANY ICU setting.

I am new to this site and so far I very grateful for its existence! =) I am a nursing student and I am trying to create a brain that would be beneficial for my clinical rotation this semester. I am in the neuro icu. Any example worksheets or advice for working in this department would be greatly appreciated!
