I use the report sheet of all my assigned residents to plan my time.
On the sheet I write all the times blood sugars are due on my diabetics, as well as when they are due long acting insulins. I also note who needs BPs, vitals, charting etc.
I go through the TAR which is organized by room and on the back of my sheet write out who is due what treatment.
I organize the OTCs in the medcart the same way every time I work so I know exactly where they are. I also make sure its stocked with all the OTCs I'll need as well as make sure I've got enough MedPass, water, and applesauce before I start my medpass.
Then I divide up my day by medpasses and accucheks. In between I do treatments and charting.
Oh, and I don't take a lunch. I might take a couple of 10 min breaks.
I prefer the 12 hr shifts I work on the weekends because I have time to get everything done. The 8 hr shifts go by too quickly.
