need ideas for report process

Hello fellow medical people,,, I have been assigned to a project at work and need some ideas. My manager wants me to come up with ideas why the report process from shift to shift is sometimes not effective enough, ie; information doesn't get passed on, things get forgotten, etc... We already use a Kardex which helps but the way our Kardex is designed , its too busy. So that is one thing I can work on. Also, we use SBAR for reporting. We tried tape recording in the past but it really didn't work because too much was missed and then you couldn't ask the staff that left because they were already gone. So if any of you have some report system that works and is effective please let me know what your ideas are!!!!! Thank you!!!!

It kind of depends on what kind of unit you're working. Can you tell us? We had a half page sheet of paper on the front of every chart in labor and delivery with spaces for the information that was the most usually necessary. Where I work now there's a list of five things we make sure we cover on each patient before leaving report. In a less specialized unit, I could see that it could be difficult to come up with a one-size-fits-all solution.

They shouldn't have been leaving until care was safely transferred to the next nurse. At 2 facilities I worked at that use taped reporting, it is to cut out all the side talk that goes on that drags out report. You made a concise taped report, but the shift ready to clock out has to wait until you have listnened to report, then they come see you and give a quick update on anything that may have changed after they taped, and ask you if you have any questions before they leave. I think this process has saved a lot of time.
