Need ideas for nursing flow sheets

I am on a committee at our facility that is reworking our nursing flow sheets. We currently use a checkoff sheet, and the graphics are on another page, and the long notes on yet another page. We are trying to incoporate all this onto one multi-page flow sheet. Anyone know of any nursing sites that may have examples of nursing flow sheets and charting. We chart way to much now in long hand, and we are trying to get the charting down to charting by exception and the flow sheets. Thanks for any help.

Oh, I forgot to mention this flow sheet is for a general medsurg floor. We see a lot of colon resections, TAH, appys, cholys and your general medical conditions such as pneumonia, COPD, CHF, pyleonephritis and such.

I would be glad to mail you an example of our flow sheet. It is from an ICU though. We get everything on one piece of paper. granted it is equal to 6 sheets of paper in a 2 X 3 grid. But it works wonderfully. I could probably get you an example of the SAC version also.

What I might try to do for you is post a picture of the forms on my picture web site.

Basicaly they both have graphics on front, and narrative and pca and pain and restraint and interventions and assessments space onthe back.

No seperate forms to deal with.

Hi dees. I think looking at the ICU format is a great idea. I'm not sure you'll be able to follow it verbatim, because typically med-surg areas of the hospital separate the graphic sheet from nurses notes. It sounds that in almost all clinical settings, paperwork has increased. If you work in one of the areas that take care of patients until discharge, I believe this is particularly true. The home health agency I work for, uses front and back nurses notes, there are so many documentation rules and regs required at different levels including agency requirements.

At some point in the future, hopefully you and your hospital will consider computerized charting. Best wishes.
