first year nursing student i need help writing a nursing diagnoses of diarrhea and include at least 3 biophysical, 3 psychosocial, and 3 educational diagnoses. Help Please:madface:

i'm trying to make some sense of what you are asking for. i understand the part about the nursing diagnosis of diarrhea. i am giving you weblinks to two online webpages that have information about this specific nursing diagnosis. they both have all the nanda information (definition, related factors and defining characteristics [symptoms]) as well as ideas for goals and nursing interventions.

however, you also want 3 biophysical, 3 psychosocial and 3 educational nursing diagnoses as well? in order to do that more information about the patient is needed. any diagnosis is based upon the symptoms that the patient is having. what are your patient's symptoms? i am only taking it at your word that this patient has the symptoms that gives him/her the diagnosis of diarrhea in the first place. definition of diagnosis: the resulting decision or opinion after the process of examination or investigation of the facts. that applies to all diagnoses whether they are nursing diagnoses, medical diagnoses or a diagnosis made by a plumber about the pipes in your house or a car mechanic about the engine in your car. they always make an examination first and then consider the abnormal signs and symptoms that they found (in nursing, nanda calls these defining characteristics) and use those abnormal signs and symptoms as the basis for their decision of what the problem is (in nursing, nanda calls this the nursing diagnosis). this is very simply problem solving. and, care planning is nothing more than problem solving that has been given a fancy name, fancy directions and a series of steps to follow.

there are two threads on all nurses that have information on writing care plans:

if after reading through that information you still need help, please ask. however, i cannot help you determine any nursing diagnoses at this point without your posting a list of your patient's symptoms. you must follow the sequence of steps in this order when care planning:

  1. assessment (collect data from medical record and by doing a physical assessment of the patient)
  2. nursing diagnosis (make a list of the abnormal assessment data, match your abnormal assessment data to likely nursing diagnoses, decide on the nursing diagnosis to use)
  3. planning (write measurable goals/outcomes and nursing interventions)
  4. implementation (initiate the care plan)
  5. evaluation (determine if goals/outcomes have been met)
