...9.D.Ice and elevate right lower extremity and right upper extremity
...5..E. Circulation, Movement, sensation (CMS) check q1h
...1..F. NPO
...2..G. IV fluid D5 45 NS at 100 ml/hr
...7..H. Morphine sulfate 1 to 2 mg IV q 4-5h PRN

prioritization is done by the patient's most important needs. keep in mind that the care plan is a problem solving process, so each nursing diagnosis is actually a patient problem. you list the problems in the order of which is most important of needing attention first. most instructors suggest prioritizing by maslow's hierarchy of needs. the hierarchy from most important to least important is as follows:
[*]safety and security needs (in the following order)
[*]love and belonging needs
[*]self-esteem needs
[*]self-actualization
so looking at this (above)what would need to go first....which could mean loss of limb, what would be most important to be dome first? how important is it and may indicate how often it needs to be done.....if you were this patinet waht would you care about?
3. prioritize the following orders from the most to the least important, and be prepared to explain the priorities you assigned.
...3..a. vital signs(vs) per routine
...6..b. neurologic check q2h (sensation intact? what could that indicate?)
...4..c. turn, cough and deep breath (tcdb) and incentive spirometer (is) q2h while awake
...9.d.ice and elevate right lower extremity and right upper extremity (could help with swelling and circultaion/sensation)
...5..e. circulation, movement, sensation (cms) check q1h (alterations could cause loss of limb?)
...1..f. npo
...2..g. iv fluid d5 45 ns at 100 ml/hr (needs hydration for npo)
...7..h. morphine sulfate 1 to 2 mg iv q 4-5h prn we know what that is for
...8..i. cefazolin (ancef) 880 mg iv q6h (infection)
