Hi everyone,
I'm new to this and love reading all your posts and stories. I'm in my second semester of an ADN program and need some help on a time management worksheet.
I need a worksheet for report/patient care. I have found a few on line, but I'm not real sure how effective they are. I do not currently work in a patient care setting and I need some samples/examples of what others are using.
I'm in my psych rotation right now, but want to be prepared for the next rotation, which will be here very soon! I'd appreciate it if anyone would like to share what they use.

I have always used the report sheets given by the hospital I worked in, then adjusted them to my own needs. I am out of the hospital setting now, but I will look around for a blank one. Pm me if you are intrested.
Thank you. If and when you have a chance, I would appreciate looking at what you use.

Hello All!
I am new to this forum, but I would like to make a suggestion....there are really two different mind-sets when it comes to the "brain sheet". When one is a student, there is a lot of information that will be asked of one that will not really be necessary to carry around on paper in the real world of hospital nursing. However, the student worksheets really ensure that you miss absolutely nothing in your assessment of patients. I attended and graduated from SUNY Orange in Middletown NY and I noticed that their student assessment form is posted here. I highly recommend it to any student. If a student uses this tool to assess patients, absolutely nothing will be missed. Hope this helps!
