Hi
I may have posted a topic a few months ago. However I really need some help with understanding the medication calculation formula when setting up for both heparin and insulin infusions.
If anyone knows of a good book, or website that has this, or are working with these types of infusions occasionally and have the know how. I would so be grateful with your help.

What type of concentration are you using for the heparin and the insulin infusions? Not sure if you use the exact formula that we do..........
For our heparin, they are normally pre-mixed with either 20,000 or 25,000 units per 500 ml bag. What do you use, then I can provide you with a simple table.........
Same thing for the insulin........many facilities here use 1 unit = 1 ml. What do you use? (Mixed 100 units in 100 ml of 0.9% NS)

I think at the moment we use the ampoules of heparin 5000units/0.2ml and then add normal saline to syringe. I think hospitals in Brisbane Australia they use 20ml syringe to put on syringe pump (ml/hr). I am not sure if Brisbane hospitals are using the premixed heparin solution as of yet?????
I am just slightly puzzled with working out the dose and ml/hr for both Insulin and Heparin solutions via syringe pump. As well as how much drug is needed to make up the rest in normal saline solution.
I hope this makes sense as to what I am requesting, if not I will need to try and rethink my question.
