Please, help I don't understand these things..I am good with care plans but not this mapping stuff....HELP...

I don't know if this will help you or not, but, think of mapping like this. All of the info you would put in a care plan e.g. nursing dx, Subjective/Objective Data, Meds, Hx, Goals, Outcome Criteria, ....Assess, Treat, Teach, Prevent....okay, all of that in separate little boxes or bubbles (your design) ...the only thing I don't include on my actual map is rationales...and if you have to have those listed do it on a separate paper. There is a great website I found that is very user friendly once you are ready to put it into map form ... www.smartdraw.com . Look under concept maps and chose create your own.
So, in a nutshell, the map is kind of a connect the dots thing. It takes all of the elements of the patient/pt. nursing care to create the big picture.
