Hi please help. I have an 85 yr old Spanish speaking patient admitted dialysis problems, (clotted fistula) for 4th stage esrd (on dialysis 5 yr). Dementia problem with self care daughter cares for, decreased appetite, fatigue, missed dialysis so hyperkalemia, hypertension
My nursing diagnosis s are.
Knowledge defeciet rt language barrier aeb needing interpreter services.
Self care rt confusion secondary to dementia aeb experiences forgetfulness, cannot complete adls, pt not knowing where he lived and what year it is.
I need help with third. I have
Ineffective breathing pattern rt fatigue aeb alterations in depth of breathing, rr of 12, and not being able to assess breathing easily.
What do you think?!! I need help! Thanks

I feel like there are more pressing issues than the risk of ineffective breathing. If she's hyperkalemic, what is she at risk for? What are some things you need to be watching/assessing?
