Need clinical practicum sites near Buena Park, Ca.

I need some potential clinical practice sites to complete my NP program. I need to work with an FNP, or MD. I need a family practice or clinic rotation, I need a long term care rotation, and 2 emergency department or urgent care rotations. Please let me know if you have any leads on these types of rotations or anyone who might be willing to help.
