need advice for going private @ home

Hello I had recently started an assignment at UCLA hospital as a LVN sitter for a pt., the pt has a alight addiction to anti-anxiety pills and pain meds, the pt is being released home and the family want me to cont. care at home as a private nurse(not thru my agency),they want ,e to occasionally flush her picc line a monitor her clear liuid diet guidelines I really want to enforce a contact before I start care, that states I wont be working under my license, since I wont be working w/ an agency, is that possibl?
