I posted this in the NCLEX area but didn't get any responses, thought this would be a better area to post it!

I sent an email to the BON asking my questions to the email address they listed for application questions, but over 2 weeks later and they haven't responded yet, so I figured I'd ask here!

For education, does it only want the nursing school you went to? Not high school? If if you went to college previously for something, like MA or NA?

For employment history, do I only list employment that is related to nursing? It states, "If you are applying for an RN: Are you currently employed as an RN? If so, please identify employer and dates of employment." It makes it sound like I should only include employment history related to nursing or the medical field.

For criminal history, it asks about what you've been CONVICTED of. Not, what you're original charges were. I have 2 previous charges. They were both sealed (expunged). I was convicted of a misdemeanor and a traffic violation (what they say is misdemeanor of-und degree). In criminal history, in regards to expunged crimes, it says, "This includes crimes that have been expunged IF there is a direct and substantial relationship to nursing practice." I figure they all can have a relationship to nursing so better to just say yes and be honest than try to figure out if it has a "substantial relationship to nursing practice." But what I am confused about is...

Then application then takes me to an "Attachments" page, where it says, "If applicable, upload the Attachments for your license application..." The area where it says, "Add Attachment," says "Compliance Supporting Document. Compliance - Provide: Explanation of the events including the data/country/state of incident; certified copy of indictment/criminal complaint/plea/journal entry from the court(s). A copy of the court docket or case summary does not meet this requirement." What would be the compliance supporting document? I was given court documents when the cases were sealed. They told me to keep them because once sealed those would be my only records of it. They each include a page that has the original complaint filed with handwriting and stamps on it, court proceeding paperwork with handwriting and stamps on it, and a certified journal report (which lists dates going through the proceedings and what happened during those proceedings). Should I attach those?

So, it says certified copy of indictment/criminal complaint/plea/journal entry - So, I can submit the certified journal report. Should I just submit all the paperwork they gave me when the records were sealed?

For the explanation, do I just write something myself explaining the charges and submit that with the paperwork?

I really hope someone can help me with some info! I am still waiting to hear back from the board about my questions and can't do anything until I finish my darn application!

Since my convictions were expunged (sealed) the only paperwork I have is what they gave me that day. They said to make sure I go down to the court desk and get them to give me copies of the paperwork because after that day there would be no records of it to obtain, so I went down and the lady gave me a packet of papers and that was it. So, I just don't know if I send that whole packet in they gave me (it has like the police report and journal entries and some random pages with court stamps and writing on it) or if I just send the journal reports?

What state are you in? I had to send certified court documents. I also had to have my transcripts only from nursing school sent straight to the board. Try to send them everything. I they ask for, so if you have the expungement orders send those I know some people uploadedthe docs, but I honestly just mailed everything.

Compliance docs would be proof of rehabilitation or proof of completion of probation, those type of things. Drugs test, if applicable or na/aa meetings.

I'm in Ohio. They didn't ask for any transcripts from school. At least not on the application.

What are expungement orders? I went to an attorney, he filed whatever he was supposed to (he did send me a copy of the letter he sent asking for expungement - well, in Ohio it's called sealing a record but it's the same thing), and then I went to court and the judge granted it. He said "make sure you go downstairs to the court clerk and get copies of your paperwork because after today you won't ever be able to get them again." So, I went to the court clerk and she gave me 2 packets. 1 for each charge. Neither of them were drug related. 1 charge was dropped down from a misdemeanor to a traffic charge because it was related to a license plate and the other charge was dropped down to a misdemeanor and was related to a rent-a-center TV that got stolen when my place got broken into. So, I don't really have proof of rehabilitation - in my expungement case, the lawyer said my proof of rehabilitation was that they were both over 10 years ago, I have not been in any trouble since then, and have graduated nursing school. It says on my court documents about probation until fines were paid, but I didn't actually have to do anything for probation and I paid the fines and that was it.

I guess I'll just send in the whole packet for each one and they can see it all!!

For the letter about the charges - am I just straight to the point with the facts? Short and quick?
