Most Embarassing thing Ever Done

Hey all, what is the most embarassing thing that has happened to anyone while at work? This can be either patient, nurse, suit, or even a visitor. So long as it fun and clean.

One day I was watching the car wash where I work and a lady's shorts split and fell! A co-worker was nice and walked up to her car and gave her a moving blanket to cover herself. When they fell, she immediately hopped back into her car for "cover".

Of course, if you know the names of anyone involved, please keep them secret so as not to embarass them further.
