Med Reconciliation...used to hate it...now love it!

When we first started the med reconciliation sheets it was a real pain. Especially getting the docs to do the discharge reconciliation. Now most of them ask us to go ahead and print them for them. I find as charge nurse that the reconciliations make my job much easier. When I do DC or transfer paperwok all the meds transfer in automatically, no need to type them all out individually anymore which was very time consuming, especially on nursing home transfers. Additionally on nursing home transfers I used to have to hand write all of the transfer orders. That was such a huge pain. Repeat admits are easier too because we have quick access to home meds, only have to verify if anything has changed. yeah! for med recon !!
