Ok, I made another mistake at the assisted living facility where I work. I gave a narc that was double the dose that was prescribed. What happened was a resident was OOF for awhile and came back and almost all her orders were the same as before she left. I knew she always got a sleeping pill st HS med pass and I noticed he med bottle was'nt in the cart so I put it in before I passed meds. I checked that it was the correct med, but did'nt pay attention to the dose. Later I was updating the monthly MAR and when copying down the med noticed the dose did'nt look right. So I checked the bottle and sure enough, the MD ordered half the usual dose for some reason. So I filled out a med error sheet and turned it in, but I work night shift so the DON has'nt seen it yet. I'm a little worried since I had two med errors last month, although they were kind of trivial imo since they were actually errors in coping over the MAR's in which the wrong per hour was transcribed on one prn med and I ordered the wrong dose on another one (both residents had numerous changes to their orders recently and I did'nt realize we were supposed to check the chart before doing the next month MAR's because nobody ever told me.
Do you guys think I will be written up or get in a lot of trouble over this? I'm thinking it might be better since I reported it and was being honest and I might not have been caught had I not reported it as I know the other nurses have made mistakes (like not giving a narc and signing it out, ect.), but didn't want to worry about somebody catching it later. Its the worst timing also as we are all under probation as the company recently changed owners and there was a major mistake by one of the other nurses recently that State may be coming in for an emergency visit to check things out. I feel I'm finally doing better as a nurse and learning and not making as many mistakes, but always seem to mess something up at the most inopportune time.
