Measurable, Timely Outcomes

my patient is postop orif to the hip. my selected nursing dx's are:

1. Ineffective Physical Mobility

2. Acute Pain

3. Noncompliance

4. Risk For Falls

5. Risk For Infection

i have to explain how the attainment of each goal will be measured and i'm not quite sure how to do that.

am i on the right track?

example ineffective physical mobility: outcome - patient is free of complications of immobility as evidenced by intact skin, absence of thrombophlebitis and normal bowel pattern.

how do i make that measurable and timely?

When we do these they have to be met by the end of our shift.

So pain - goal is pt. reports a 3/10 or lower on the scale by the end of the shift via meds and whatever else you will do/did (distraction, etc)

Does your CI want that kind of long term goal? It doesn't sound very timely the way you wrote it.

Look at -

ambulation (based on what PT cleared him/her for)

risk for infection - C&DB, incentive spirometry, and also address the incision

Oh - noncom? Why?

he's a grumpy old man, lol... doesn't like being bothered with having to take medications and monitoring vitals ...
