so i need to make a pretend schedule and i am in need of major help. i have been working on this for days and cannot figure out how to do it correctly.....PLEASE HELP......
THIS IS THE SENARIO-------
Tasks for day shift, 7-3:30 pm of wing 1 Sunnyside LTC
4 SHOWERS- each of these residents transfers with assistance of 2 staff members. Each of these residents needs to be weighed, have BP checks and have a complete linen change on shower day. One resident states that she feels dizzy and has congested sounding cough.
16 partial baths- ten residents are able to wash own face and hands when set up. On the remaining six, one has developed a rash over his whole body, one needs glucose monitoring x2 on a day shift and four are confused and incontinent of urine and feces. Each of these residents needs one assistant to transfer and ambulate.
1 Complex Dressing change- for a resident on a Clinitron bed. This resident is transfered by a lifting device and requires total care.
1 PEG TUBE intermittent feeding-with commercial tube feed product x2 and drug administration x2 on day shift. This resident is confused and requires assistance of 2 staff to ambulate.
STAFF AVAILABLE---
2 LPN's, one with 5 years experience and one with 6 months experience
1 CNA who has worked at Sunnyside 10 years
1 CNA who has 7 years experience and worked at Sunnyside 6 months
1 CNA who was sent from a temp agency to fill the position of a nursing assistant who has the flu. She completed her training course 2 months ago.
As a charge nurse, how should you make the assignments for the day?
You have just completed assigning team members when one of the CNA's states that she is feeling warm and faints. She has a temp of 104.2F and needs to be sent home. Reassign the Residents of Wing 1.

I am not familiar with LTC....but I'll give it a try.
Does the charge nurse take any patients? Which CNA goes home?
Each LPN would have 11 patients. 1 complex total, 2 showers/8 partials (1 incontinent)
1 total, 2 showers/8 partials (3 incontinent)
I would assign the experienced LPN with the facility........ the complex drsg change and clinitron bed, glucose monitoring, and rash, 1 incontinent. (complex patient to the experienced staff)
The other LPN would have the tube feeding, dizzy/cough, 3 incontinent.
The three aids would split care.
The CNA with 10 years at the facility would help the new LPN (1 incontinent). (6 total)
The CNA new to the facility but with experience would help the experienced LPN
(1 incontinent). (6 total patents)
The agency CNA would care for the walkie/talkies/partials and 2 incontinent. (10 total patients)
When an aide has to go home I would take 4 continent walkie talkies/self care partials. and distribute the rest equally.

There's not really a right or wrong answer here. The purpose of the assignment is to get you thinking from a supervisory standpoint. If YOU are the one in charge, what are YOU going to do when things go wrong?
Your instructor doesn't expect you to know exactly what assignments are the best. She wants to get you thinking from a different perspective. It's not easy, and there isn't going to be a perfect solution. The other point of the assignment is to help you recognize that sometimes there are tough decisions that have to be made by management that affect the employees. There might not be a way to make everyone happy. Sometimes there is no good way to make the assignment, but everyone has to join together to do the best they can.
So when you're doing this assignment, just put yourself in the shoes of the charge nurse. Think about your patients's needs and the skills of the staff and try to match them up the best you can. Don't worry if it's not perfect. There is no perfect schedule here.
