M0102 and M0104

i have read and reread the verbage on these 2 items and still need some clarification.

example: if you call a pt to schedule soc for lets say 11/16 and patient says "oh no you need to come on 11/21" so you call the MD and tell them that the patient does not want a visit until monday, does that monday date become the date you enter in M0102 as MD specific???? different people i work with are interrupting it in different ways...so i need to come to "my experts" here.
