Los angeles area Help! Need physical and mental health examination

hi fellow nurses ,

I have been on probation for two years and I'm finishing  up my last year.  unfortunately I found out through my probation monitor that the doctor I was seeing for my physical and mental health examination retired a few months ago.  are there any nurses in the Los Angeles area that can refer me to a provider who can conduct the exam for me? I would appreciate any input.
