Looking for Patient Acuity Worksheet

When I worked out in California we had a patient acuity worksheet that was great. It was very simple and rated patients on a scale of 1-5. Of course that was many years ago and I no longer have access to the worksheet. The hospital unit I am working for now is thinking of implementing staffing by acuity but the only worksheet they have is more geared to an ICU not a floor unit. I was wondering if anyone had an acuity worksheet they would be willing to share with me. I am desperate and can't phathom how a two page patient acuity worksheet that focuses on ventilators, a-lines, and multiple drips is going to make my job any easier.

Really?? No one uses patient acuity worksheets where they work... I've done a search on the board but have only found one. I'm really hoping someone can help.
