Lifestyle Change project ???????

One more month and school is over and now I got the worst assignment to do.... about myself!

To go back to school, while having a family with two small kids was a huge change in my lifestyle....not necessary in a positive way.

Yes, I am looking forward to work as a nurse soon, but before I have to manage this hurdle.

I just hope someone can please give me an idea on this project.

It includes a

comprehensive assessment relating to determinants of health (health changes I want to make or already making),

a nursing diagnosis,

SMART goals,

interventions and rationales as well as

evaluation techniques. On top of that I have to find

3 theory based suggestions for clients making life style changes.

The biggest problem is, there is no real health change for me to think of. I do not smoke or drink, I eat healthy and enjoy my hobby endurance riding, which I had not too much time lately but it always has been the best exercise program for me. I have a caring husband and two girls I am very proud of. What I do need is a bit more time for my family and sleep, which I believe almost everybody lacks.

I even thought about to MAKE UP a life style change plan just to get this paper done.

Please, please does anybody have some ideas?

go to the center of a taber's cyclopedic medical dictionary to the entries for nursing where there is a 7-page assessment and assess yourself. or, use one of these:

nursing diagnoses are based upon what is abnormal in an assessment. no one is completely normal. and, if you are, then we use the wellness diagnoses which are to enhance (better) yourself. here is a list of them:

- one of my doctors has recently written a book about alzheimer's disease and says that a lifelong lack of sleep and being under regular stress are the two biggest contributing factors (that we can change) to the development of alzheimer's disease in the population of the developed world and it's going to hit epidemic numbers in my baby boomer generation!

I wasn't paying attention. I guess you have to have 10 posts before the PM function becomes active.

We had horses and ponies. I got thrown off a pony when I was about 4 when he took off at a gallop. I have a spondylolisthesis that I have been told probably happened as a result of a vertebral fracture as a child. You should see the x-rays of my back. I've had a laminectomy and I get problems with one leg when I stand for any length of time. I see a chiropractor regularly. No more epidurals if I can help it.

Oh my gosh, I know exactly what you talking about. I often worked on patients with spinal injuries when I was a Physio.

Good to hear that the Chiropractor can help you. We didn't have Chiros back in Europe where I came from. The Physios did most of the therapies and we were quite successful with manual mobilization and manual therapy. A bit less rough than Chiropractors.....

I just hope that with those experiences you haven't lost you love for horses.....

I have another question regarding the life style change project. I was amazed to find so many wellness diagnoses. I didn't even know that something like that exists. Now, the difference to an actual nursing diagnosis is that I can't really hook on to an physiological/psychological problem (from my point of view)

How on earth is it possible to come up with interventions/ rationales and especially evaluation since this goes more toward health promotion. It sound almost like science fiction and in the real work world you never have an ideal condition.

Or, maybe I have trouble to think outside the box.

I checked the internet for and back to find the NANDA II taxonomy. No luck so far.

Hopefully I will find something in the library tomorrow.

here is one wellness diagnosis in the ackley/ladwig constructor that is online: [color=#3366ff]readiness for enhanced knowledge (specify)

these are pretty much learning and improvement diagnoses. keep that old nursing process in mind. assess what your strengths are with wellness before you go in and start doing any improvement.

do you have a medical dictionary? both taber's cyclopedic medical dictionary and

mosby's medical, nursing, & allied health dictionaryhave the nanda taxonomy printed in the back of them. i haven't seen a copy of stedman's, so i don't know if it has this information or not. whether or not they have the wellness diagnoses listed will depend on how old the edition of the dictionary is.

i'm looking at my copy of ackley/ladwig. readiness for enhanced sleep was added to the taxonomy by nanda in 2002. they list the related factor as "desire to improve sleep". they list 3 suggestions (these are not exclusive):

  • awaken refreshed and is not fatigued during the day
  • fall asleep without difficulty
  • verbalize plan to implement sleep promotion routines
  • (page 1114, nursing diagnosis handbook: a guide to planning care, 7th edition, by betty j. ackley and gail b. ladwig)

interventions for it that ackley/ladwig have listed are things like doing a sleep history to identify routines and patterns (i was thinking that a dietician would do the same thing with someone when evaluating them before creating an eating plan), keeping a sleep diary, a whole bunch of different interventions to help a person get to sleep (obvious stuff for people who have trouble falling asleep). time management might also be something to consider. i would imagine that if a classmate has this book you could xerox these 4 or 5 pages. you could also search the internet for info on sleep and find information. as i was saying about the diet above. . .a lot of things follow similar patterns. . .the same kinds of strategies you employ for one thing cross over and work pretty well for something else. if you need taxonomy information for any diagnoses i will give them to you.

Yes, I do have Ackley/Ladwig (6th) and also Mosby's Dictionary 2002.....and I FOUND it! Yippee!!!!

I would have never even thought about to check the appendix of those books. Besides, before I started the project, I wouldn't even have known, WHAT to look for.

You are welcome. I wasn't really aware of what was in the appendix of those books until I was answering a question for someone one day and I went through the appendix and was stunned myself. I buy a copy of the taxonomy from NANDA every year and mark it up with all kinds of notes. I know that people buy the Ackley/Ladwig book primarily because of Section I, that cross index. When I saw the cross index in Taber's I almost fell off my chair. The Mosby dictionary has one too, but it's really not as good.

So, I guess we'll be seeing an enhanced Arablover by graduation, huh? :wink2:
