I'm wondering what people's personal practices generally are regarding lidocaine prior to placing male Foleys.

I've used it intermittently over the last 4 years.
I've had two cases with A&O men who'd had a prior Foley who were very apprehensive and said it was a horrible experience. In both cases, using the lidocaine and then waiting for 20-30 min, the men said that they hardly felt it.
There are a few published studies that find that it significantly reduces discomfort when placing foleys.
