hello,

i am filling out my application for my GN. the directions state to always use the same form of your name. it asks for first, middle, and last name.

the testing center request that i have my name on licensure paperwork exactly as its written on my id.

heres my question, since my name on my id is written first name, middle intital, last name, should i fill out my application that way? meaning i would only write my middle initial and not my whole name???

i know this is probably a silly question, but i am scared if i do something wrong everything will be rejected. please help

i hope i am understanding you correctly.

if you id has your name as first name: j middle: k last name: m

yet the paper work ask you fill out your name in reverse, just fill out in the appropriate section exactly as the id you will give on the day of exam:

last name:m first name:j middle: k

as well if you have a hyphenated last name or two last names, on your id, make sure you put the entire last names.

make sure you get your name right because this will cause problems/delay later with getting your att, testing, checking results/ status, background checks, etc. :imdbb: this was a problem for me, with how the school had my name and how my complete legal name was on my id.
